NDVIP ContinuumCare is a non-clinical wellness membership. This notice explains what information we collect, how we use it, and the choices you have. We share your wellness information only at your direction.
1. Information we collect
We collect the information you provide when you enroll and use the Service: your name, email, mobile phone number, and account credentials; the wellness inputs you share through check-ins (such as hydration, nutrition, movement, sleep, and weight you choose to report); your preferences and settings; and billing details processed by our payment provider. We also collect basic technical information such as device/browser data and the IP address used when you accept these terms.
2. How we use your information
We use your information to operate and personalize the Service — to send your check-ins, reminders, and progress reports; to compute your wellness summaries and scores; to provide SmartCare coordinator support; to process billing; and to maintain the security and integrity of the Service. We do not use your information to make medical decisions, and we do not sell your personal information.
3. How your information is shared
Your wellness information is shared only at your direction. For example, if you choose to share an optional progress summary with a provider you designate, we will send that summary. We use trusted service providers (such as our hosting, messaging, email, and payment vendors) to operate the Service; they may process information only to provide their services to us. We may disclose information if required by law.
4. SMS & email communications
With your consent, we send recurring SMS and email related to your membership, including daily check-ins, reminders, weekly progress reports, and messages from your SmartCare coordinator. Message frequency varies.
Mobile phone numbers and SMS consent are never shared with or sold to third parties or affiliates for marketing purposes. No mobile information is shared with third parties or affiliates for marketing or promotional purposes. Information may be shared with subcontractors in support services, such as customer service, solely to provide the Service; those subcontractors are prohibited from using mobile information for any other purpose.
Message and data rates may apply. You can opt out of SMS at any time by replying STOP, or reply HELP for assistance. You may also adjust email and message preferences in your account settings. Carriers are not liable for delayed or undelivered messages.
5. Data security & retention
We use reasonable administrative, technical, and physical safeguards designed to protect your information, including encryption in transit and at rest and role-based access controls. No method of transmission or storage is completely secure. We retain information for as long as your account is active and as needed to comply with our legal and recordkeeping obligations.
6. Your choices
You may review and update editable preferences in your account settings, opt out of SMS, and request access to or deletion of your information, subject to applicable law and our recordkeeping obligations. Some changes (such as identity or billing details) are handled through your SmartCare coordinator.
7. Changes to this notice
We may update this Privacy Notice from time to time. The version and effective date above will be updated, and material changes will be communicated through the Service or by email.
